Canthal Tilt Explained: What It Is, Why It Matters, and How to Improve It

What Is Canthal Tilt?

Canthal tilt refers to the angle formed by a line drawn from the medial canthus (inner corner of the eye) to the lateral canthus (outer corner of the eye). When the outer corner sits higher than the inner corner, this is called positive canthal tilt. When the outer corner sits lower, it's negative canthal tilt. Equal height is neutral.

It is one of the most frequently cited metrics in the PSL / looksmaxxing framework because it strongly influences perceived eye shape, intensity, and overall facial attractiveness.

Positive Canthal Tilt — The "Hunter Eyes" Look

Positive canthal tilt creates what is commonly described as "hunter eyes" — an upward, almond-shaped eye appearance that projects alertness, intensity, and dominance. High-PSL male faces almost universally exhibit some degree of positive canthal tilt.

Features associated with positive canthal tilt:

• Eyes appear narrower and more almond-shaped from a frontal view
• Creates a naturally intense, focused expression at rest
• Associated with higher testosterone/sexual dimorphism readings
• More common in East Asian, Southern European, and Middle Eastern facial phenotypes

Negative Canthal Tilt — "Sad Eyes"

Negative canthal tilt causes the outer corners of the eyes to drop, creating what is perceived as a sad, tired, or submissive expression even at rest. It is consistently rated less attractive in male faces and is a common failo in MTN and LTN-tier subjects.

Negative canthal tilt often co-occurs with:

• Round, "prey eye" shape
• Scleral show (visible white below the iris)
• Lower perceived assertiveness/dominance
• An overall "soft" facial impression that conflicts with high PSL readings on other metrics

How Is Canthal Tilt Measured?

Canthal tilt is measured in degrees from horizontal. A tilt of +3° to +8° is considered ideal in most looksmaxxing frameworks. Neutral is 0°. Negative tilt begins below -2°. Anything beyond -5° is a significant failo that can drag the PSL score by 0.3–0.7 points independent of other metrics.

Can Canthal Tilt Be Changed?

Yes — canthal tilt is one of the few major facial metrics that can be surgically altered with high precision:

Canthopexy (Non-Surgical / Minimally Invasive)

A canthopexy tightens the lateral canthal tendon without cutting, elevating the outer corner by 1–3mm. Minimally invasive, performed under local anesthesia, 1–2 week recovery. Best for mild negative tilt or as a preventive measure against age-related drooping. Cost: $2,000–$5,000.

Canthoplasty (Surgical)

A canthoplasty reconstructs the lateral canthus, allowing more aggressive repositioning of the outer corner. Can create 3–8mm of elevation. More downtime (2–4 weeks), higher cost ($4,000–$10,000), but more permanent and dramatic results. The primary surgical intervention for converting negative to positive canthal tilt.

Lower Lid Blepharoplasty

Removing excess lower lid skin and fat can improve the appearance of negative tilt and reduce scleral show, even without altering the canthus itself. Often combined with canthoplasty for maximum effect.
